If you have any questions, please do not hesitate to ask. Many times, your nurse can answer questions immediately. A patient representative is also available if you have comments or concerns about your care or the services we provide.

Care Management

The Department of Integrative Care Management (ICM) provides coordination of patient care in the hospital and in the outpatient setting. The ICM staff, consisting of registered nurses and social workers, works with patient care team members including physicians, registered nurses, patient care technicians, and financial assistance counselors. All these individuals work together to offer assistance and information to patients and family members regarding personal or family crises, adjustment to illness, discharge planning, in-home nursing care, extended care facility placement, and community resources that may be available after discharge from the hospital.
