After Your Stay

At Miami Valley Hospital (MVH) we know that your health and general well-being are a priority, and not just while you’re a patient here. That’s why MVH provides a variety of support and educational resources to help meet your health needs after your stay at MVH is over.

Going Home

Your physician will generally let you know a day or two before you will be dismissed from the hospital. Before your dismissal, it’s a good idea to make a list of any questions you have about your follow-up care.

When your doctor has approved your discharge, your nurse will provide written instructions about your dismissal and follow-up care. A staff member or volunteer will escort you (by wheelchair if necessary) to the dismissal area and will help you into your car.

Special Needs

The MVH Social Services staff is available to help you plan for any special care you may require after you leave the hospital. Social workers can assist you in making the necessary arrangements for additional nursing care, therapy, or special medical equipment for your home. Referrals for counseling and support for personal, family, or financial problems is also available. If you would like to see a member of the Social Services staff, you may call extension 2251 or ask your nurse to contact the department.

Home Care

Fidelity Health Care offers a comprehensive range of home health care services throughout the Miami Valley region. They understand that recovery does not end with your discharge from the hospital. Fidelity’s clinicians are experienced professionals trained to meet the specific needs of each individual patient in the comfort of his or her own home. Fidelity’s services include:

  • RNs, LPNs, home care aides, and rehabilitation therapists
  • Medical social workers
  • Infusion therapy
  • Private duty
  • Oxygen therapy
  • Home care medical equipment

Fidelity also offers disease management programs for congestive heart failure, respiratory disease, and other specialty areas.

Patient and Family Education

During your stay, we will offer you ways to learn about your condition, treatment, medicines, and care after you leave the hospital. We will also inform you about services you may want to use in your community. While you are here, feel free to watch the health programs on our television system’s patient education channels.

Once your doctor has signed your discharge orders, a nurse will go over your instructions with you and your family. If you have any questions, please make sure you ask your nurse or doctor.
